Refer a patient
For doctors, hospitals, clinics and case managers. Send us the referral and a coordinator will contact the patient or family to arrange care at home.
After you submit
What happens next
- 1. You receive a referral reference number.
- 2. A coordinator reviews the referral and contacts the patient or family.
- 3. Care arrangements are made inside our care management system.
- 4. Visit records are documented and shared with those the patient authorises.
Not for emergencies. Home care is not a substitute for emergency care. If someone is in immediate danger, has chest pain, severe bleeding, difficulty breathing or is unresponsive, go to the nearest emergency department or call emergency services.
